The short answer
Use remarcar to reschedule and horário to mean a time or appointment slot. Give the appointment date or reference first so the provider can locate it. Then explain briefly that you cannot be home at the original time.
Ask to change the appointment politely
Offer one or two alternatives when you can. A specific option is easier to answer than a general request to come whenever you are available.

Confirm the visit details that matter
